Prostate Supplements for Men Over 50: What the Research Says, and 4 Formulas Compared
By OakMan Guide Editorial Team Updated 10 min read
The short answer
Most prostate supplements combine a few plant extracts: saw palmetto, pygeum, stinging nettle and, in the liquid formulas, seaweed. Saw palmetto is the best known and well tolerated, pygeum has encouraging short trials, and a saw palmetto and nettle pairing has the best-documented results for night-time trips. Choose a formula that tells you what is inside, in a format you will take every day, with a money-back guarantee that gives you two to three months.

Saw palmetto, pygeum, nettle and seaweed: what each one is known for, and how to pick a formula you will be happy to take every day.
What happens to the prostate after 50?
The prostate is a walnut-sized gland that sits under the bladder and wraps around the tube you pee through. It tends to grow as men age. When it grows enough to squeeze that tube, doctors call it benign prostatic hyperplasia, or BPH. "Benign" is the key word: it is not cancer.
It is also common, and it gets more common every decade. According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), BPH affects 5 to 6 percent of men ages 40 to 64 and 29 to 33 percent of men 65 and older.
The symptoms NIDDK lists will sound familiar to a lot of us:
- Trouble starting a stream, or feeling like the bladder never quite empties
- A weak or stop-and-start stream, or dribbling at the end
- Getting up at night to pee (we wrote a separate guide on why you wake up to pee at night after 50)
- Needing to go often, or suddenly and urgently
We go through all eight common signs in our article on enlarged prostate symptoms after 50.
For many men it shows up in their fifties, when the night trips begin, and the doctor's word is "mildly enlarged." That is common at this age, and it is a good reason to read labels instead of guessing.
What do the main ingredients bring?
Prostate formulas are built from a short list of plants. Here is what each one is known for, ingredient by ingredient.
Saw palmetto: the best-known prostate herb
Saw palmetto is in all four formulas we have reviewed, and it is the most studied prostate herb there is. The National Center for Complementary and Integrative Health calls it "well tolerated" and says it was used in studies for up to 3 years, with mild, infrequent side effects. It does not appear to affect PSA readings, so a daily habit does not get in the way of your yearly checkup. The trials and the label tips get their own write-up in our article on saw palmetto for the prostate.
Pygeum: a consistent signal in short trials
Pygeum comes from the bark of the African plum tree. A Cochrane review of 18 trials with 1,562 men found men taking pygeum were more than twice as likely to report that their symptoms improved, with night-time urination down 19 percent and peak urine flow up 23 percent. Side effects were mild and similar to placebo.
Saw palmetto plus nettle root: the pairing with its own studies
A European combination of saw palmetto and stinging nettle root extracts, called PRO 160/120, was followed in four controlled trials with 922 men. In a 2014 re-analysis published in the World Journal of Urology, night-time bathroom trips dropped 29 percent with the combination versus 18 percent with placebo. If getting up at night is your main complaint, this is the pairing with the most research behind it. Our article on pygeum vs saw palmetto vs nettle root lines the three herbs up with the numbers from each trial.
Seaweed: kelp, bladderwrack, nori and wakame
The two liquid formulas lean on sea vegetables. Kelp is a rich source of iodine, an essential mineral, according to MedlinePlus, and it is a traditional ingredient in coastal kitchens. Seaweed is what gives Prostadine and ViriFlow their character.
Cat's claw, graviola and the supporting cast
The capsule formulas add cat's claw, soursop (graviola), raspberry and, in one case, green tea. They are the supporting players, there to round out the herb list next to saw palmetto and pygeum.
| Ingredient | What it is known for | Found in |
|---|---|---|
| Saw palmetto | The best-known prostate herb; well tolerated and used in studies for up to 3 years (NCCIH) | All four |
| Pygeum | Night-time urination down 19% in a Cochrane review of 18 trials | ViriFlow, Prosta Peak, Prosta Defend |
| Saw palmetto + nettle root | A specific extract cut night trips 29% vs 18% on placebo (World J Urol) | Prosta Defend (both herbs) |
| Seaweeds | Kelp, nori, wakame and bladderwrack; kelp is a rich natural source of iodine (MedlinePlus) | Prostadine, ViriFlow |
| Cat’s claw, soursop, raspberry | Supporting herbs that round out the blend | Prosta Peak, Prosta Defend |
A prostate formula is a mix of plants that have a long history side by side, and that is the idea behind all four of these. Some men notice fewer night trips, and the money-back guarantee lets you see for yourself.
Drops or capsules: which format fits you?
Once you strip away the marketing, the four formulas we have looked at fall into two families.
The drops family: Prostadine and ViriFlow. Both are liquids built on sea vegetables (nori, wakame, kelp and bladderwrack) plus saw palmetto, pomegranate, shilajit and neem, taken as a 2 ml daily serving. Prostadine is oil-based and prints the milligrams of each sea vegetable on its label. ViriFlow is water-based and adds pygeum, citrus pectin and plant sterols to its blend. You can take them straight or stir them into coffee or juice, which is handy if you hate swallowing pills.
The capsules family: Prosta Peak and Prosta Defend. These lean on the classic prostate herbs: saw palmetto and pygeum, with cat's claw, soursop and raspberry. Prosta Defend adds stinging nettle, which puts it closest to the saw palmetto and nettle pairing from the European studies. Prosta Peak adds green tea and describes a blend of more than 20 ingredients. If you already take a daily pill organizer, these slot right in.
How do the 4 formulas line up?
Here is the side-by-side, all four in one table. Prices are per bottle as listed on each official page when we checked; bigger packs cost less per bottle, and stores can change prices at any time.
| Formula | Format | Key named ingredients | Guarantee | Per bottle |
|---|---|---|---|---|
| Prostadine | Drops, 2 ml a day (oil-based) | Seaweeds, saw palmetto, neem, pomegranate, shilajit | 60 days | $49 to $69 |
| ViriFlow | Drops, 2 ml a day (water-based) | Seaweeds, saw palmetto, neem, pygeum, pomegranate, plant sterols, shilajit | 60 days | $49 to $79 |
| Prosta Peak | Capsules | Saw palmetto, pygeum, cat’s claw, green tea, raspberry, soursop | 180 days | $49 to $79 |
| Prosta Defend | Capsules, 2 a day with a meal | Saw palmetto, pygeum, stinging nettle, cat’s claw, raspberry, soursop | 90 days | $49 to $79 |
In one line each: Prostadine is the best-known liquid, ViriFlow is the water-based drop with pygeum added, Prosta Peak gives you the longest window to decide, and Prosta Defend is the one that lists nettle next to saw palmetto.
Each of the four sells through its own official store, which is where the current packages, the money-back guarantee and customer support described here apply. That is where the buttons below lead.

Capsules with saw palmetto and nettle, 90-day guarantee.
How do you choose a prostate supplement? A 6-point checklist
- Find the supplement facts. The more the maker tells you about each ingredient, the better.
- Look for the plants with the best signal. Pygeum, and saw palmetto paired with nettle root, have the most to show.
- Pick a format you will stick with. Drops if pills are a chore, capsules if you already take a morning handful.
- Look for a long money-back window. Sixty days or more gives you time to judge results.
- Make sure it is a one-time purchase. All four of these are.
- Plan a fair trial. Two to three months, a simple note of how many times you get up each night, and a decision before the guarantee runs out.
As the official pages advise, ask your doctor first if you take medication, are pregnant or nursing, or have a medical condition. All four are easy to live with, so pick the format you will actually remember to take.
What else helps with night-time bathroom trips?
A supplement works best on top of the basics. These are the habits NIDDK recommends for managing BPH symptoms, and none of them cost a dime:
- Drink less before bed, and before long drives or events.
- Avoid or limit alcohol and caffeine, especially later in the day (here is what to cut first).
- Empty your bladder completely when you go, and do not hold it for long stretches.
- Stay physically active. A daily walk counts.
Frequently asked questions
What is the best supplement for an enlarged prostate?
There is no single best one, and that is good news: you can choose by what suits you. Look for a formula that lists what is inside, comes in a format you will take every day, and has a guarantee long enough to judge it. Saw palmetto, pygeum and nettle root are the plants with the most prostate research behind them.
How long do prostate supplements take to work?
Give any formula two to three months. The pygeum studies ran about 64 days on average and the saw palmetto and nettle combination studies ran 24 weeks, so a week tells you very little. A simple count of your night-time trips gives you a number to compare.
Are prostate drops better than capsules?
Neither format is better by itself. Drops like Prostadine and ViriFlow are built around seaweed and can go into coffee or juice. Capsules like Prosta Peak and Prosta Defend lean on classic prostate herbs. Pick the format you will actually take every day.
Do I need to buy a six-month supply?
Not necessarily. The bigger packs are cheaper per bottle, and a longer supply matches how long the studies ran. Buy as much as you will use inside the money-back window, and the official store shows the packages that are current.
Why order from the official store?
That is where the current packages, the money-back guarantee and customer support are the producer's own. You get the product as the company makes it, with the offer and the refund window written on the order page.
What makes a good prostate supplement label?
A published Supplement Facts panel, named ingredients with the plants that have research behind them, a clear daily serving, a one-time payment and a guarantee of 60 days or more. All four formulas in this guide check those boxes.
